Medicare Part D is Medicare's prescription drug coverage. It is available in two forms: as a standalone Prescription Drug Plan (PDP) added to Original Medicare or a Medigap policy, or integrated within many Medicare Advantage plans (known as MA-PD plans). If you already have a Medicare Advantage plan that includes drug coverage, you generally cannot add a standalone Part D plan on top — the coverage is already built in.
Every Part D plan has a formulary: the official list of covered medications organized into tiers, typically from tier 1 to tier 5. Your tier determines how much you pay per prescription. Lower tiers generally include generic drugs and carry lower copays; higher tiers correspond to brand-name or specialty medications. Because formularies vary from plan to plan, comparing which plan covers your current medications is essential before you choose.

The best time to enroll in a Part D plan is when you first become eligible for Medicare — what is known as the Initial Enrollment Period. If you do not enroll at that time and do not have creditable coverage from another source such as an employer, the federal government may impose a Late Enrollment Penalty (LEP): a permanent surcharge added to your monthly Part D premium for as long as you have Medicare.
You can also enroll or switch plans during the Annual Enrollment Period (AEP), which runs from October 15 to December 7 each year.
